Appointments
Our office has an Open-Access policy.  We strive for same-day and next-day appointments.  We make it easy to book your appointment:

  • Online directly
  • Through email
  • Calling our office

Arriving more than 10 minutes late to your appointment may result in rescheduling.
Cancelling your appointment with less than 24 hours notice, or not showing up as scheduled, may results in a $25 rescheduling fee.

 

Prescription Refills
In an attempt to reduce the need for refill requests, the doctor will try to make sure patients have enough refills to last their next recommended follow-up (typically 3 months).  Please remind the doctor of refills at every visit.
We ask that all patients be seen for an office visit prior to authorizing any type of medication refill, with the following exceptions:
- Prescriptions filled at the on-site pharmacy (Provincial Pharmacy) may receive a limited-supply refill by contacting the pharmacy directly

- Phone request for refills may be subject to a $15 fee
- Controlled substances (opioids, stimulants, sedatives) will only be refilled during an office visit

 

Lab and Diagnostic Test Results
We do not discuss test results over the phone (with the exception of INR results for patients on Coumadin).  We ask all patients to come back for a return office visit to discuss results.  Results are generally available 1 week after the test has been performed.  If you contact the office about results, we will only confirm whether or not results have arrived.
To help serve our patients better, we ask the patients choose one of the following options at the time of testing:
  1) schedule a follow up appointment for one week or later
  2) ask the front desk to contact you when results are in so you may be seen in a timely manner

 

 

Services not covered by OHIP
There are a variety of services offered that are NOT covered by OHIP.  Patients are expected to pay at the time of service.  Special arrangements for financial hardship are made on a case-by-case basis at the discretion of the physician.

After-Hours Access
We have limited after-hours access, typically one evening a week and a weekend day.  If you need medical assistance after our offices have closed, please use the following:

  • if the symptoms are serious, please call 9-1-1 immediately, do not attempt to contact the office
  • if symptoms are mild, you can try the AAFP’s flow chart to determine if you need to seek further care:

  • Telephone Health Advisory Service 1-866-553-7205.  They will let you know whether you'll need to see the on-call physician or proceed elsewhere 
Controlled Substance
Dr. Zalzala takes controlled substances very seriously.  The medications are typically used for severe pain, to calm the nerves, or for ADD/ADHD.
All patients who Dr. Zalzalza feel are appropriate for controlled substances prescribed by him must must sign a Controlled Substance Agreement - a contract between the patient and the doctor.
